Claim: 1. A method of accessing target tissue in a lung, the method comprising: locating a target site on an airway wall, wherein the target site is in a location that allows for creation of an opening leading to the target tissue and for advancement of an elongate instrument through the opening to the target tissue; creating an opening through the airway wall at the target site with a hole-making instrument, the opening leading to the target tissue; advancing the elongate instrument through the opening created in the airway wall; providing an atraumatic tipped dissector, the dissector positioned within the elongate instrument; tunneling from the opening in the airway wall to the target tissue with the elongate instrument and the dissector positioned therein; removing the dissector from the elongate instrument such that the elongate instrument provides access to the target tissue for performing monitoring, diagnostic or therapeutic procedures through the elongate instrument; and performing a procedure on the target tissue with a procedure instrument through the previously positioned elongate instrument leading to the target tissue.
Claim: 2. The method of claim 1 , wherein the target site is in a location that allows for advancement of an elongate instrument to the target tissue along a substantially straight path.
Claim: 3. The method of claim 2 , wherein the target site is determined by considering one or more of the following factors: a size of the airway; an outer diameter of a scope used to advance the elongate instrument in the airway; space available within the airway to articulate a scope so that the scope working channel can point towards the target tissue; a position of blood vessels in the lung; a location of pleural surfaces within the lung; a location of other airways in the lung; and/or a location, size, and/or shape of a target tissue.
Claim: 4. The method of claim 1 , wherein the target site is in a location that allows for advancement of an elongate instrument from the opening in the airway wall to the target tissue along a substantially straight path.
Claim: 5. The method of claim 1 , wherein the target site is in a location that allows for the creation of a substantially straight tunnel or channel leading from the target site or opening to the target tissue.
Claim: 6. The method of claim 1 , wherein locating a target site on the airway wall comprises determining a direction or orientation of a tunnel or channel in space, the tunnel or channel leading from the airway wall to the target tissue in a substantially straight line.
Claim: 7. The method of claim 1 , wherein the opening through the airway wall at the target site is created by advancing a piercing member to the target site and through the airway wall.
Claim: 8. The method of claim 7 , further comprising dilating the opening with a balloon catheter or other expandable device.
Claim: 9. The method of claim 1 , further comprising articulating the elongate instrument within the airway such that a distal end of the elongate instrument points in a direction facing the target tissue before the elongate instrument advances through the opening.
Claim: 10. The method of claim 1 , wherein the dissector is a dissecting catheter having a rounded tip.
Claim: 11. The method of claim 1 , further performing imaging to guide the elongate instrument to the target tissue.
Claim: 12. The method of claim 11 wherein the imaging method used is comprised of at least one of: fluoroscopy, computed tomography, positron emission technology, magnetic resonance imaging, or ultrasound.
Claim: 13. The method of claim 1 , wherein the target tissue is a tumor or diseased tissue.
Claim: 14. The method of claim 1 , wherein the procedure instrument is a biopsy tool, and the procedure is to obtain a biopsy sample through the elongate instrument.
Claim: 15. A method of accessing target tissue in a lung, the method comprising: advancing a flexible elongate instrument to a target site in an airway of the lung; creating an opening through the airway wall at the target site with a hole-making instrument; advancing the elongate instrument through the opening; urging the elongate instrument and a dissector positioned within the elongate instrument forward through lung tissue, thereby creating a tunnel from the opening in the airway wall to the target tissue; removing the dissector from the elongate instrument such that the elongate instrument provides access to the target tissue for performing monitoring, diagnostic or therapeutic procedures through the elongate instrument; and performing a procedure on the target tissue with a procedure instrument through the previously positioned elongate instrument leading to the target tissue.
Claim: 16. The method of claim 15 , wherein the elongate instrument is advanced to the target tissue along a substantially straight path.
Claim: 17. The method of claim 15 , wherein the elongate instrument is advanced from the opening in the airway wall to the target tissue along a substantially straight path.
Claim: 18. The method of claim 15 , further comprising locating a target site on an airway wall, wherein the target site is in a location that allows for creation of an opening leading to the target tissue and for advancement of the elongate instrument through the opening to the target tissue.
Claim: 19. The method of claim 18 , wherein the elongate instrument is advanced to the target tissue along a substantially straight path.
Claim: 20. The method of claim 15 , further comprising dilating the opening with a balloon catheter or other expandable device.
Claim: 21. The method of claim 15 , further comprising articulating the elongate instrument within the airway such that a distal end of the elongate instrument points in a direction facing the target tissue before the elongate instrument advances through the opening.
Claim: 22. The method of claim 15 , further comprising leaving the elongate instrument in place to provide access to the target tissue for performing monitoring, diagnostic or therapeutic procedures or for delivering instruments or tools to the target tissue.
Claim: 23. The method of claim 15 , further performing imaging to guide the elongate instrument to the target tissue.
Claim: 24. A method of accessing target tissue in a lung, the method comprising: selecting a target site in an airway of the lung wherein the target site is in a location that allows for creation of an opening through the airway wall, the opening leading to the target tissue, and wherein the target site is in a location that allows for advancement of a flexible elongate instrument through the opening to the target tissue along a substantially straight path; advancing the elongate instrument to a target site in an airway of the lung; creating an opening through the airway wall at the target site with a hole-making instrument; advancing the elongate instrument through the opening created in the airway wall; urging the elongate instrument and a dissector positioned within the elongate instrument forward through lung tissue, thereby creating a tunnel from the opening in the airway wall to the target tissue; removing the dissector from the elongate instrument such that the elongate instrument provides access to the target tissue for performing monitoring, diagnostic or therapeutic procedures; and performing a procedure on the target tissue with a procedure instrument through the previously positioned elongate instrument leading to the target tissue.
Claim: 25. The method of claim 24 , further performing imaging to guide the elongate instrument to the target tissue.
Claim: 26. The method of claim 25 wherein the imaging method used is comprised of at least one of: fluoroscopy, computed tomography, positron emission technology, magnetic resonance imaging, or ultrasound.
