الوصف: The purpose of this quality improvement project was to implement the use of a printed individualized after-visit summary (AVS) in a northeast Ohio small practice for patients receiving workers’ compensation with chronic non-cancer pain (CNCP). Patients receiving workers’ compensation face numerous hurdles before receiving treatment for his or her injury. There are multiple moving pieces to a workers’ compensation claim, which adds additional stress to the patient visit making it difficult to remember what was discussed during the office visit. After synthesizing the literature findings and determining specific components of an AVS that promote the best patient outcomes, the AVS was individualized and administered by the provider monthly to 26 workers’ compensation participants. Data was collected that measured three outcome variables: patient satisfaction, pain, and adherence to treatment plan. Time Point 1 was prior to initiation of the AVS and time Point 2 was after the third individualized AVS was administered, or 2 to 3 months following Time Point 1. The project sought to answer the question: Does the use of an individualized printed AVS improve pain, adherence to the treatment plan, and patient satisfaction? While results failed to illustrate statistical significance in patient satisfaction, findings revealed that after individualized AVSs were given, there was significant change in pain and patient adherence. Key recommendations to improve adherence to a complex treatment plan includes customizing the AVS. Clinical suggestions of an effective AVS include using a suitable literacy level, verbally reviewing the AVS, excluding unnecessary text, highlighting focal items, and encouraging patient questions.
