الوصف: 應用研究, 研究期間 10208~ 10307, 研究經費 1350 千元 ; 雖然水中有很多病原菌(例如退伍軍人菌、綠膿桿菌、非結核分枝桿菌、包氏不動桿菌、氣單孢菌屬與曲黴屬真菌等等)不會造成一般大眾的感染,但是對於住院的病人,特別是免疫不全病人,健康威脅非常大。近來衛生署疾病管制局與世界衛生組織紛紛發表最新的醫院用水安全的指引,如果醫院水中發現這些水生病原菌,醫院必須採取措施,以避免繼續散播。醫院供水系統消毒似乎是治本的最好方法,但是消毒系統都非常昂貴,醫院無法獨力負擔,進行全時的醫院用水消毒。本研究試著提出一個經濟有效的方法,透過間歇式的供水系統消毒,來達到水生菌,特別是退伍軍人菌與綠膿桿菌的移生控制。本研究將以前瞻方式進行,藉由(1)找出供水系統有水生菌移生的醫院;(2)評估該醫院適合運用銅銀離子消毒法,或是二氧化氯消毒法;(3)探討最佳的間歇式加藥消毒的頻率與停機的時程;(4)調查停機之後,這些水生菌移生恢復的時間。本研究的參與醫院,將從研究者之前國科會計畫進行的採樣醫院中挑選,並遵照標準作業程序,進行醫院供水系統的水生菌採樣、培養與鑑定。本研究的順利執行,將可以提供世界各國醫院參考,找出經濟有效的醫院供水系統消毒方法,以保護住院病人安全,以及保障公共衛生福祉。 ; Although microorganisms such as Legionella, P. aeruginosa and mycobacteria, Acinetobacter, Aeromonas and Aspergillus species do not appear to represent a health concern through water consumption by the general population, including most patients in health-care facilities, they may be of concern for severely immunosuppressed patients. Recently, both Taiwan CDC and WHO published guidelines in monitoring of Legionella and other waterborne pathogens to prevent the nosocomial transmission of these pathogens to hospitalized patients. Disinfection of hospital water system has been proved to be the most cost-effective method in preventing nosocomial legionellosis and other waterborne infections. However, the cost associated with the disinfection which is the biggest barrier when implementing such proactive measure. Thus, we propose a proactive approach to determine the cost-effectiveness of intermittent disinfection program using copper-silver ionization and chlorine dioxide systems. The study will be conducted in the following prospective fashion: (1) identifying the study hospitals (hospitals with high Legionella and P. aeruginosa colonization).
