| الوصف: | Introduction: Infertility affects 10% of the population of fertile age. Improvements in reproductive medicine have overcome most causes of infertility, with the exception of uterine factor infertility, due either to absence of a uterus (congenital or acquired) or presence of a non-functional uterus. No population-based study has been done to accurately determine the size of this group of women but in the UK it has been suggested to include 12,000–15,000 women. Uterus transplantation (UTx) may become a treatment for this large group of permanently infertile women. Material and Methods: Aiming to develop techniques of UTx in the baboon and asses long-term function, 16 female baboons underwent autologous UTx, including oviducts and ovaries. The ovaries were included to be able to assess menstrual cyclicity, as a measurement of surgical success. The animals were divided in 2 groups (n = 6/10) where the techniques of surgery and flushing were developed further in group 2. Arterial ends (internal iliac artery) and venous ends (Group 2 including patches of caval or left kidney vein) were joined forming one artery and one vein of the specimen during back table preparation. The transplant was flushed with HTK solution and after cold preservation transplanted end-to-side to the left external iliac artery (Group 1) and end-to-end to the left internal iliac artery (Group 2) with venous anastomosis end-to-side to the left external iliac vein (both groups). Results: Short time survival of the animals was 88%. Out of the surviving animals 75% (66% in group 1 and 80% in group 2) showed resumed ovarian cyclicity. Regular menstruation was only showed in group 2 (60% of the animals). Six animals were exposed to timed mating for eight completed menstrual cycles, but pregnancy did not occur. Our current research is towards allogenic UTx in the baboon and so far six transplantation attempts including retrieval surgery from living donor and transplantation of the uterus alone has been done.
