The current study explored defining characteristics of Black women’s experience of traumatic childbirth via semi-structured interviews to learn how this group is uniquely impacted. This study addresses three questions. First, what are Black women’s experiences of childbirth trauma? Second, are these themes unique to Black race and thus different from the current literature/findings normed on white experiences? Third, how can psychologists more efficiently engage Black women in therapy and tailor psychological treatments to include more culturally appropriate elements that will be most efficacious? Using Colaizzi’s Seven-Step Descriptive Phenomenological Method, qualitative analysis of participant interviews revealed ten central themes divided amongst five categories related to Black women's experiences of childbirth trauma. The following categories and themes include determinants of trauma (lack of provider communication, non-observation of patient and human rights, and inadequate response to pain), patient health behaviors and literacy (health literacy, patient advocacy, and awareness of Black maternal mortality), racial impacts on care quality, emotional experiences, and consequences of childbirth trauma (decreased interest in subsequent children and diminished postpartum recovery). Analysis of the distribution of symptom inventory ratings (PPQ and EPDS) after traumatic childbirth experiences did not statistically differ between Black women and known white norms. Additional exploration of the present childbirth trauma literature indicate that experiences of inadequate response to pain, awareness of Black maternal mortality, and racial impacts on care quality are unique to Black women. Themes related to dissemination and implementation were also identified using the RE-AIM model. Relevant themes include reach (messages to increase treatment-seeking behaviors, ideal therapist qualities, decreasing resource obscurity, and systemic barriers to treatment), adoption (help-seeking via communal, state, and federal ... |
