Terminal patients deserve to go through their last moments of life receiving all the support that generates comfort and does not prolong agony. The objective of this review article is to analyze the available bibliographic evidence on the limitation of therapeutic effort in terminally ill patients and the main limited or adequate therapeutic measures. The method of obtaining information is through bibliographic searches in Health Sciences databases: Elsevier, Scielo, Pubmed, in addition to other medical and nursing journals and university library repositories in Google Academic. The selection criteria were national/international documents with a summary and full availability of the free text, published in the last 5 years (between 2018-2022), in English and/or Spanish. 82 similar documents with the subject of study were preselected and after exhaustive reading, 28 were used. The information was organized into 3 categories: Main measures of limitation or adaptation in the therapeutic effort to terminal patients, Limitation of Therapeutic Effort from the point of view bioethics, Experience and level of knowledge of Health Personnel applying the Limitation of Therapeutic Effort. It is concluded that the main measures applied are the order not to start cardiopulmonary resuscitation and withdrawal of mechanical ventilation. The bioethical principles of autonomy, non-maleficence and beneficence are the most violated. The main difficulties in endof-life care in the ICU are academic-cultural difficulties, structural-architectural difficulties, and psycho-emotional difficulties. The level of knowledge of health professionals about the protocolized application of LET is insufficient ; O paciente terminal merece passar por seus últimos momentos de vida recebendo todo o suporte que gere conforto e não prolongue a agonia.
