INTRODUCTION AND AIMS: Even though HSP in adult patients is usually associated with a good prognosis, some patients may present severe visceral involvements. To date, there is no comparative evaluation of treatment in such patients.The aim of this study was to compare the risk/benefit ratio of steroid (STE) alone or associated with cyclophosphamide (CYC) in severe HSP. METHODS: This multicenter, prospective, randomized open-label trial included 54 adult patients with biopsy proven HSP and severe visceral manifestation including endocapillary and/or extracapillary proliferative glomerulonephritis (class 3 or 4).Patients were assigned to receive either STE alone (group STE, n = 29) or associated with CYC (group CYC, n = 25).The primary end point was the complete remission of the disease at 6 month (M6). Secondary endpoints included patient and renal survival, the presence of renal abnormalities. Safety was assessed at M6 and M12. RESULTS: No significant difference was shown between the 2 groups. Only 3 patients in each group displayed active skin lesions at M12 and none demonstrated active visceral involvements. At M12, patient survival was 79 % in group STE vs. 96% in group CYC. Renal function, proteinuria and hematuria were not different between the 2 groups. Of note, in group STE and CYC, renal function remained stable during the follow-up and patients experienced a significant decrease of daily proteinuria between inclusion and M12 (3.2±1.1 and 0.3±0.2 g/24h vs. 3.6±1.2 and 0.6±0.1g/day respectively). The incidence of hematuria decreased also in both groups during the follow-up (STE 89 to 36% vs. CYC 96 to 50% respectively). Safety data, including diabetes mellitus and severe infection, was also similar between the 2 groups. CONCLUSIONS: We report here the first largest study of prospective therapeutic intervention for severe HSP in adults.
