الوصف: The aim of this study is to develop a model of care enabling elderly people with dementia continue to live at home or in familiar environment (Aging-in-place). We visited 18 small community-based multiservice agencies providing such care in Fukuoka and Kumamoto prefectures and extracted 100 items from semi-structured interview data that were qualitatively analysed according to the similarity of meaning. These activities were further categorized statistically to 77 items satisfying internal consistency and factorial construct validity. In order that those small community-based multiservice agencies provide care with the concept of Aging-in-place, 5 factors sould be considered: (1)Developing a system elderly people with dementia can receive proper medical care on the round-the-clock bases; (2)Making these agencies more familiar to elderly people and their families, and service professionals such as care managers as the places providing end-of-life care; (3)Supporting elderly people to keep ties with local communities; (4)Making use of multiplicity of the agencies in order to provide end-of-life care at home or in familiar environment; (5)Enhancing cooperation among community-based care service providers and making their services more accessible. ; departmental bulletin paper
