| الوصف: | Introduction: Data suggest that atrial pacing from Bachmann's Bundle (BB) reduces intraatrial conduction time, improvessynchronization of left and right atrial contraction, and may reduce the riskof atrial fibrillation compared to conventional pacing from the right atrialappendage. A detailed understanding of BB's anatomy is extremely important forcorrect lead implantation in this area. Methods: We studied the 24 specimensof cadaver hearts. The techniques were as follows: morphometry, micropreparation, sheet plastination (E12), polymeric embalming of specimens (S10) and plastination with a room temperature process. Results: BB represents a group ofthe muscular fibers, which is the main way for impulse conduction from theright atrium (RA) to the left atrium (LA) as illustrated in figure 1 (A. Left-anterior view; B. Superior view). Three sources give rise to the bundle:fibers from the sinus node and the terminal crest, fibers from the anteriorwall of the RA, fibers from the septum near superior vena cava (SVC). Theclosest right atrial enodcardial position relative to BB is the upper part ofthe intraatrial septum just beyond the SVC. In order to approach this locationwith a stylet driven lead, it is necessary to configure the stylet with smalldistal curve. We suggest curving the distal part of the stylet close to a fullcircle with diameter about 1,5 cm or to place a 1,5 cm right angle curve at theend of the stylet. |
