الوصف: | Background and aims: ; Recent studies revealed distinct composition of gut microbiota in subjects with Parkinson’s disease (PD), which suggested the involvement of gut microbiota and microbiota-gut-brain axis in the pathogenesis of PD. Nonetheless, the roles of gut microbiota at the early stages of synucleinopathy, REM sleep behavior disorder (RBD) and their relatives, and its interactions with the prodromal markers of PD are still unclear. In this study, we aim to identify the colonic bacterial compositions in subjects with early PD, RBD, first-degree relatives of RBD (RBD-FDR), and healthy controls, and further correlate the abundances of these microbiota with clinical biomarkers. ; Methods: ; This is a case–control study based on our prospective longitudinal family cohort of RBD and controls. Early PD was defined as onset of PD symptoms less than five years, while the diagnoses of RBD were confirmed by video-polysomnography. Subjects underwent face-to-face clinical assessments. Fecal microbial community composition was generated by sequencing the third and fourth variable regions (V3- 4) of the microbial 16S ribosomal RNA gene. ; Results: ; A total of 228 controls, 125 RBD-FDR, 154 subjects with RBD and 34 subjects with early PD were recruited into study. Microbial analysis was performed in 64 controls, 83 RBD-FDR, 80 RBD and 15 early PD subjects. ; 1) Neurodegenerative biomarkers. The prevalence of functional constipation (6.3 vs 13.3 vs 46.7 vs 67.6 %, P < 0.001), olfactory impairment (15.8 vs 22.5 vs 73.6 vs 84.8 %, P < 0.001) increased from controls, RBD-FDR, RBD to early PD. Total likelihood ratio (LR) of prodromal PD were highest in RBD, followed by RBD-FDR and controls. RBD-FDR with RBD features (RBD-FDRᴿᴮᴰ⁺, n = 28) reported more constipation (P = 0.007), higher total LR of prodromal PD (P = 0.04) and prevalence of probable prodromal PD (P = 0.01) when compared with RBD-FDR who were devoid of RBD symptoms (RBD-FDRᴿᴮᴰ⁻, n = 97). ; 2) Microbial composition.
