الوصف: | Over the past two decades, allogeneic haematopoietic stem cell transplantation (HSCT) has established its position as part of the treatment for many childhood malignant and non-malignant diseases of the bone marrow and immune system. At the Helsinki Children’s Hospital, approximately 20–25 allogeneic stem cell transplants are performed annually for paediatric patients. Complications following allogeneic stem cell transplantation typically relate to infections and the development of graft-versus-host disease (GvHD), which is caused by differences in human leukocyte antigens between the donor and the recipient. GvHD can be classified as acute or chronic. One of the most challenging forms of chronic GvHD is chronic GvHD of the lungs, which causes significant morbidity and mortality. There is limited research on the diagnosis, treatment or monitoring of pulmonary GvHD manifestations, with existing research primarily focusing on the adult population. This study aimed to investigate the clinical presentation and diagnosis of GvHD of the lungs, most importantly bronchiolitis obliterans syndrome (BOS). As pulmonary function changes related to BOS are often irreversible, it is important to identify patients at an early stage to plan the appropriate follow-up of pulmonary function tests after allogeneic stem cell transplantation. There is a significant decline in lung function tests early post-HSCT in children with chronic respiratory symptoms after allogeneic HSCT. The histological findings of BOS correlated closely with pulmonary function tests, showing early signs of obstructive lung function and a mosaic pattern in lung high-resolution computed tomography scans. Paediatric allogeneic HSCT patients with severe chronic GvHD of the lungs and obstructive lung function face a substantial risk of sleep-related breathing disorders, indicated by a ventilation–perfusion mismatch, increased breathing effort and hypoventilation during rapid eye movement (REM) sleep.
