Background:Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome affects 1 in 4.000-5.000 live births, in which there is a failure of development of the uterus and vagina. Anorectal malformation are rare, however, this condition presents early at birth or in infancy. Case Report: 28 years-old-female with MRKH presents a complex perianal fistula, previously underwent unsuccessful surgical treatment. A magnetic resonance shows a transsphincteric fistula associated with abscess. Surgical treatment was developed in association with Plastic Surgery: fistulectomy and confection of a neovagina to repair both abnormalities in two surgical times. Actually, our patient has no perianal symptoms. Discussion: there are two types of MRKH, type I o typical: isolated uterovaginal aplasia, and type II or atypical: incomplete aplasia commonly associated with other malformations. MRKH with anorectal malformation is seldom reported, rectovestibular fistula is by far the most frequent defect of anorrectal anomalies seen in girls. There is no agreement about correcting both anomalies at the same time or in two times. ; Introducción: el síndrome de Mayer-Rokitansky-Kuster-Hauser (MRKH) afecta a 1/4.000-5.000 recién nacidos, consistiendo en un fallo en el desarrollo de útero y vagina. Las malformaciones anorrectales asociadas a este síndrome son raras, propiciando la aparición de síntomas más precoces en la infancia. Caso clínico: mujer de 28 años con MRKH con fístula perianal compleja intervenida en varias ocasiones sin conseguir un tratamiento definitivo de la misma. En resonancia magnética se aprecia trayecto transesfinteriano junto con colecciones anteriores y posteriores a aparato genital. Se interviene de forma conjunta en dos tiempos con Cirugía Plástica, realizándose fictulectomía con confección de neovagina mediante colgajos Singapore. La paciente se encuentra actualmente sin sintomatología perianal.
