| الوصف: | 原著 背景:現行の医師臨床研修制度が2004年度に開始されてから約10年が経過し,わが国の医育制度として定着しているが,制度開始時と比較すると,医学部生の医師臨床研修制度に対する認識が変化していると予測される.そこで,医師臨床研修制度開始時と現在の医学生の医師臨床研修制度に対する認識がどのように変化したのかを明らかにするためにアンケート調査を行った.方法:2002年度と2014年度に東邦大学医学部医学科5年生に対して行ったアンケート調査結果より,医師臨床研修制度に対する認識の変化を検討した.結果:医師臨床研修制度の開始から10年間で,制度の内容についての理解は深まったものの,制度が開始された経緯やプライマリ・ケアの重視,研修への専従といった理念への認識が低下している傾向がみられた.また,研修医が自由に研修診療科を選択できる選択期間は,技能・知識を修得する期間というよりも,将来の専攻科を決定するための期間と考えている学生が増加した.さらには,1つの診療科の期間を短くして,より多くの診療科で研修を行いたいと考えている者が増加していた.結論:2002年度と比較し2014年度では本来の医師臨床研修制度の理念への認識が薄れ,臨床研修を将来の専攻科を決める場と認識する学生が増加している可能性が考えられた.Background: It has been more than a decade since the postgraduate clinical training system began in Japan, and most medical students are now familiar with this system. However, student perceptions of this system might be changing. In 2014, we surveyed student opinions on the postgraduate clinical training system. To identify changes in student perceptions, we compared those responses with data from a similar questionnaire study, done in 2002. Methods: Similar questionnaires on the clinical training system were distributed in 2002 and 2014 to fifth-year medical students at Toho University. The results of the 2014 survey were analyzed and compared with those from the 2002 survey. Results: Although the postgraduate clinical training system is now widely recognized, there is little understanding of the context of its development and fundamental ideas, such as the emphasis on primary care and training. Recent students tended to consider the free-selection period - during which they can freely select their department - as a trial period for their future specialty. Thus, they now focus on quickly having experience in a number of departments rather than on acquiring knowledge and techniques. Conclusions: The fundamental ideas of the clinical training system are being forgotten because medical students now tend to regard the postgraduate clinical training system as a trial period for deciding their future specialty. |
