الوصف: | Objective: The oncologically safe application of thermal in-situ ablation techniques is limited by a lack of precise prediction of the resulting destruction. The aim of the current study was to validate a 3D planning system for in-situ ablation of liver tumors with regard to the simulation of the cooling effect of intrahepatic vessels in vivo. Methods: A liver CT scan was performed on domestic pigs. Subsequently, an applicator for laser-induced thermotherapy (LITT) was introduced near a large intrahepatic vessel and LITT was performed (28 W, 20 min). The liver was removed and the lesion was digitally measured. On the basis of the CT scan a segmentation of the liver and intrahepatic vessels was performed and a simulation of the thermal lesion was carried out. The real and the simulated lesion were compared with regard to size and shape. Results: 12 thermal lesions were induced in 8 animals. The mean deviation of the real to the idealized lesion was 27.3%. The volume of lesions invivo was 7.39 ± 1.79 cm3, that of calculated lesions was 7.41 ± 1.68 cm3. The morphometric analysis showed a spatial correlation of 76% between real and simulated lesions. Conclusions: (1) A 3D computer planning system was developed for predicting thermal lesions resulting from in-situ ablation. (2) This system enables a calculation of the cooling effect of intrahepatic vessels for the first time. (3) The planning system facilitates a safer application of in-situ ablation techniques. |
