الوصف: | Introduction: Parapharyngeal tumors are rare, representing about 0.5% of all head and neck tumors. Most (80%) are benign. Primitive tumors correspond to 90% of the cases (40% to 50% originating from the salivary glands and from 25% to 40% with neurogenic origin). They are asymptomatic in the initial stage. The diagnosis is made when the tumor reaches 3 cm, manifesting as an oropharyngeal or cervical mass. The clinical presentation is similar regardless the histological type.Material and methods: We reviewed the literature and reported three cases operated on the Otorhinolaryngology Department of the Hospital Garcia de Orta.Results: The reviewed cases illustrate the most common etiologies (pleomorphic adenoma, schwannoma and paraganglioma) which are clinically similar.Discussion and conclusions: However, a careful analysis of the lesion topography on clinical exam, assisted by TC and MRI (which is more specific) allow a precise surgical planning for the resection of the tumor and definitive histopathological diagnosis. |
Description (Translated): | Introdução: Os tumores parafaríngeos são raros, representando 0,5% dos tumores de cabeça e pescoço. A maioria (80%) é benigna. Os tumores primitivos do espaço parafaríngeo correspondem a 90% dos casos (entre 40% a 50% com origem em glândulas salivares; entre 25% a 40% com origem neurogénica). São na fase inicial assintomáticos, sendo o diagnóstico feito quando as dimensões ascendem aos 3cm, manifestando-se por uma massa orofaríngea ou cervical. A apresentação clínica é semelhante independentemente do tipo histológico.Material e Métodos: Revimos a literatura e relatamos 3 de entre os casos operados no Serviço Otorrinolaringologia do Hospital Garcia de Orta.Resultados: Os casos revistos ilustram as etiologias mais frequentes (adenoma pleomórfico, schwannoma e paraganglioma), que são clínica e imagiologicamente semelhantes.Discussão e conclusões: Contudo, uma análise cuidada da topografia da lesão no exame objectivo e os aspectos sugestivos na Tomografia Computorizada e na Ressonância Magnética, permitem um planeamento cirúrgico com vista à exerése completa da lesão e ao diagnóstico histopatológico definitivo. |
